Central Community Health Centre

 

Where Every One Matters

 

Central Community Health Centre is a charitable organization whose purpose is the delivery of primary healthcare to members of the communities of St. Thomas, Southwold and Central Elgin, with an emphasis on those people experiencing barriers to care. CCHC's services will complement existing healthcare in these communities and will include primary healthcare, health promotion and illness prevention. Plans for the CCHC were developed by a local steering committee and Founding Board of Directors after extensive community consultation.

 

 

QUICK FACTS


  • The Central CHC is located at 359 Talbot Street in St. Thomas and is one of 125 CHCs in Ontario.

  • The Central CHC serves residents of Central Elgin, Southwold Township and St. Thomas.

  • The CHC model of care focuses on primary health care, illness prevention, health promotion and working closely with other health cna wellness providers in the community.

  • Front-line care staffing at CHC may include, doctors, nurse practitioners, registered nurses, social workers, chiropodists, dietitian, health promoters and community outreach workers - all working together as a team to provide client care.

  • The Central CHC is one of four CHCs funded by the South West LHIN.

How are we funded?

 

The Central Community Health Centre receives its funding from the Ontario Ministry of Health and Long Term Care provided through the South West Local Health Integration Network (LHIN). The South West LHIN is responisible for, "planning, integrating and funding more than 150 health service providers, including hospitals, long-term care homes, mental health and addiction agencies, community support services, community health centres and the South West Community Care Access Centre"

 

The Central Community Health Centre has an agreement with the LHIN called an Accountability Agreement (See link below). This document explains our relationship with the LHIN and what our mutual responsibilities are.
